Responsibilities:

• Be a member of the Multidisciplinary team to evaluate social work needs of each participant.
• Develop a care plan including short and long term goals with each participant’s family and other appropriate individuals.
• Promote peer group relationships through problem-centered discussion and task-centered groups.
• Serve as a liaison between Golden Acres ADHC and the participant’s home and family.
• Serve as a liaison between Golden Acres ADHC and senior service agencies: maintain referral resource file for Golden Acres ADHC primary and secondary service areas as well as pertinent referrals outside of service area.

Duties:

• Interview and screen all referrals to determine the general appropriateness of each prospective participant for the full assessment proceeds and adult day health care participation.
• Provide referral for those not appropriate for adult day health care.
• Provide in-home assessment for all participants.
• Provide group sessions in the program
• Provide participant, caregiver and family counseling as needed
• Sign and date documentation for all services performed the day service is provided.
• Sign and date quarterly progress reports on each participant’s record
• Provide periodic reevaluation and revision of care plan
• Provide counseling and referral to other community resources.
• Provide discharge planning for discharge participants.

Work hours: Monday-Friday, 7:30am - 2:00pm

Job Types: Full-time, Contract, Temporary

Pay: $20.00 - $23.00 per hour
