An esteemed Adult Day Care Center located in the heart of the Bronx is seeking a dedicated Case Manager/Social Worker to join our compassionate team. As a Case Manager/Social Worker, you will play a pivotal role in addressing the diverse needs of our clients and ensuring their well-being within our center.

Responsibilities:

• Conduct comprehensive assessments of clients to determine their needs, strengths, and goals.
• Develop individualized care plans in collaboration with clients and their families to address identified needs and goals.
• Provide ongoing case management services, including advocacy, coordination of services, and referrals to external resources.
• Facilitate support groups and therapeutic activities to promote socialization and emotional well-being among clients.
• Maintain accurate and up-to-date client records, including progress notes, assessments, and service plans.
• Collaborate effectively with interdisciplinary team members, including healthcare professionals and community agencies.
• Monitor client progress and make adjustments to care plans as needed to ensure optimal outcomes.
• Provide crisis intervention and support during times of emotional distress or significant life changes.
• Participate in staff meetings, trainings, and professional development activities to enhance skills and knowledge.

Qualifications:

• Bachelor's or master's degree in social work or related field.
• Current licensure or eligibility for licensure as a Social Worker in the state of New York.
• Previous experience working with older adults or individuals with disabilities preferred.
• Strong assessment, communication, and interpersonal skills.
• Ability to work independently and as part of a team in a fast-paced environment.
• Proficiency in computer applications, including Microsoft Office Suite.
• Bilingual proficiency in English and Spanish is a must.
