Job Title: Case Manager

Reports to: Program Director

FLSA Status: Non-Exempt

Agency: University Consultation & Treatment Center for Mental Hygiene Inc. a non-profit behavioral mental health agency located in the South Bronx

Position: Case Manager (Bilingual a plus) 3 open positions

Job Description:

Duties and responsibilities include, but are not limited to the following:

Interviews Clients in situations involving social, emotional, financial, marital, health, or other problems to develop background and details of problems.

Assesses and evaluates cases and prepares and implements case management plans with the resident.

Counsels clients, assisting them to identify and resolve problems and make effective use of resources.

Case Manager will also advocate on behalf of tenants and assist them in securing all benefits to which they are entitled.

Maintains working relationships with staff of other agencies and institutions, homes, and facilities, and acts as liaison between clients and agency or other institution to assist client.

Encourages clients to be more independent and retain feelings of self-esteem, responsible and part of their community.

Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other task as required.

Assuming responsibility for establishing: and maintaining linkages essential to accessing. Other activities include assessment and referral, individual and group counseling, and, other assigned responsibilities that deal directly with ensuring that tenant's function at their highest level and that their supportive service needs are met.

Qualifications: Bachelor’s degree, and at least one year experience in a related field. Experience working in an apartment treatment program preferred. Computer Literacy, excellent writing and verbal communications skills needed.

Onsite and scatter site Field work required.

Shift Hours are 9-5PM; 24/7 on-call availability required.
