Job description
Patient Care Coordinator
Small privately-owned home medical equipment company looking for an experienced patient care coordinator or clinical liaison. The perfect candidate for this position is detail orientated with rockstar communication and documentation skills. This position entails communication with your patients, physicians, and coworkers to process new orders for prescribed medical devices. Reviewing with the patient where they are at in the order process, while getting needed clinical documentation from their physician. Part of the job will entail learning about new products and benefits of the treatment in order to appropriately converse with your patients about their orders. This position is directly involved with the preadmission process, establishing patient eligibility, communication with the patient or family, reviewing patient records, establishing clinical coverage based on medical necessity and explaining equipment/ treatment options to the patient. Patient care is important to us. This role works in tandem with others in the office and out on the field to facilitate proper patient care, so teamwork is important.
Job Duties include:
Gathering preadmission information, to assist in better communication of information about the patient to all disciplines.
Responsible for educating patients or their caregivers about the services and products provided by our medical company.
Serves as a point of contact, coordination, and communication with physicians and patients.
Provides oversight to the providers regarding the patient and proper assessment and treatment processes.
Reviews the patient’s medical records to obtain primary and secondary insurance payor information, and relays this to the intake department.
Oversees the prescriptions for medical equipment are filled.
Qualifications and Skills
2 years of clinical experience with patient care or health care administration preferred. Proven knowledge of and experience with ICD-10 and medical terminology. Outstanding interpersonal, verbal, and written communications skills required. Must be flexible and able to work in a fast-paced, heavy volume work environment. Demonstrated computer, prioritization, and time management skills. Experience working with health care professionals, insurance providers, or with medical device reimbursement for a start-up or new technology company or durable medical equipment setting is highly desirable.
EDUCATION/KNOWLEDGE
Bachelor of Health Administration or a related field. May consider Associates, High school diploma or general education degree GED based on job-related experience. Familiarity with medical device reimbursement from a manufacturer or DME perspective is preferred.

LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write, read, and interpret routine reports and correspondence, frequently medical in nature. Ability to speak effectively over the phone and before groups of customers or employees of the organization.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide into all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
