Description:

- Supporting the in-patient and ED patient populations

- Mostly working with adults, but occasionally some teenagers will come in.

- Working with psychiatric/mental health patients in their in-patient ward.

- Need to do mental health evaluations

- Cross continuum care coordination (clinic, provider care, urgent care, phone care, skilled care, etc)

- Support with discharge planning

- Psychosocial assessments and psychosocial interventions

- Identify opportunities to continually improve patience care, services, and population health

Additional Skills & Qualifications:

- Need to be a clinical social worker.

- Two licenses accepted: Social Worker Associate Independent Clinical License OR Social Worker Independent Clinical License

- They can accept either a associates or an independent license. If associates degree need to commit to obtain their LICSW within 4 years.

- 2-3 years of acute in-patient med/surg hospital experience

- Nice to have mental health/psychiatric patient experience

- Need to have proficient computer skills

- Exposure to an EMR system is preferred ideally EPIC (what Multicare uses)

- Ability to have organizational skills (managing the work and flow).

- MSW degree
