Position Summary The Quality Improvement Specialist focuses on value-based care arrangements and supports overall organizational quality improvement strategies. This position coordinates strategies to improve quality HEDIS measurements and patient outcomes. Patient outreach, reporting, data analysis, and documentation are key areas within this role.Core Job Responsibilities1. Uses population health quality analytic tools and payor care gap reports to recommend quality care gap opportunities.2. Performs chart audits and analyze to identify patient care gap opportunities. 3. Performs outreach to patients to provide patient education on health care preventive measures and the importance of preventive health, patient engagement with primary care and other health care providers, and encouragement of patient responsibility for self-management of health4. Contacts patients to schedule referral appointment scheduling for preventive and care gap needs. 5. Documents evidence of patient outreach, including specific quality care gaps, and the patient and provider interactions in patient’s medical records 6. Communicates with provider through in-basket messaging regarding patient care gaps; highlight for the provider, the specific needs to drive improvements in patient care, provider and patient satisfaction, and optimizing patient outcomes. 7. Uses reports to monitor trends and improvements in data collection.8. Drives improvement of provider performance by analyzing, interpreting, and communicating financial, utilization and quality metrics; provides coaching and feedback and develops targeted training to provider teams to improve quality, accuracy, and overall documentation. 9. Provides support for quality improvement initiatives and projects as needed based on organizational and department priorities. 10. Performs other duties as assigned. Requirements/Preferences Associate degree in Health Care Administration, Public Health or Social Science related field, required. Bachelor’s degree preferredMinimum of one (1) year quality improvement experience preferredBi-lingual, Spanish speaking is preferredMinimum of one (1) year experience working with multi-disciplinary teams requiredFQHC background preferred Knowledge of HEDIS/NCQA and measurement and/or reporting requirements is preferred Working knowledge of electronic health records system required: EPIC Clinical Software System preferredIntermediate proficiency in Microsoft Office Suite (highly skilled in PowerPoint and Excel) Competencies/Behaviors Normal office environment Remote working opportunity as needed Ability to sit for up to 4 hours at a timeComputer/laptop Phone/Fax/Copier/Scanner
