· Receives, evaluates, accepts, and executes the initial client intake process to assure that patient verification, assessment, teaching, and the ongoing scheduling of clinical/medical services occurs in a timely manner.
· Retrieve informed consent and release of information.
· Complete psychosocial and medical assessments
· Completes intake checklists for all new clients.
· Enters patient information into the database system including ICD-10 codes, medical history, medication profile and multidisciplinary care plan and retrieve from referral source missing information.
· Communicates professionally through multiple means with all stakeholders.
· Works closely with medical director, nurses, therapist, hospital liaisons, case managers, social workers, and other medical professionals involved in client care.
· Participate in daily team meetings.
· Use attention to detail and knowledge of regulatory compliance to ensure accurate work and follow up.
· Setup software programs and program technology for active clients (Patient Portals, Breathalyzer, iPad, Fitbit etc.)
· Strong customer service skills with the ability to work with diverse people.
· Effective organizational and time management skills
· Verbal and written communication skills
· Understanding of health insurance and various medical procedures
· Ability to juggle multiple tasks in a fast-paced environment.
· Problem-solving skills
