We have immediate multiple openings for LPNs, offering ongoing educational opportunities, competitive rates and benefits packages.

Duties and responsibilities:

  • Provides ongoing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn.
  • Following the plan of care, necessary revisions, working under seasoned RN supervision
  • Initiates and performs appropriate nursing interventions.
  • Coordinates services, informs the physician and other personnel of changes in the client's condition and needs.
  • Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
  • Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.

Job Types: Full-time, Part-time, Per diem

Pay: $35.00 - $40.00 per hour

Expected hours: 10 – 40 per week
