In this role, you will provide high-quality care to patients in the comfort of their own homes. You will work collaboratively with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans. Your typical day will include:
- Conducting patient assessments
- Administering medications
- Performing wound care
- Educating patients and families on self-care management
- Monitoring vital signs
- Providing emotional support and companionship
To be successful in this role, you must have:
- An active RN or LPN license in the state of Massachusetts.
- Minimum 1 year of experience in a clinical setting (prior home health experience a plus)
- Excellent communication, interpersonal, and assessment skills
- Strong work ethic, compassion, and a positive attitude
- Ability to work independently and manage time effectively
- Valid driver's license and reliable transportation
