KeyResponsibilities:

• IndividualizedCare with Inclusion: Provide individualized care that upholds the uniqueneeds and aspirations of individuals with disabilities. Promote inclusionwithin the community, fostering a sense of belonging and empowerment.

• CommunityIntegration: Engage in community outings, social activities, and events toencourage individuals to actively participate in community life and forgelasting connections. Provide direct service to individuals with disabilitiesthroughout the community.

• EmpowermentThrough Skill Development: Collaborate with individuals to develop lifeskills, communication abilities, and self-confidence, enabling them to takecontrol of their personal journeys.

• Person-CenteredSupport: Apply person-centered approaches, focusing on preferences,choices, and aspirations. Leverage the individual's strengths to drive growth.

• BehavioralSupport with Dignity: Implement positive behavior support strategies thatrespect individuals' dignity. Handle challenging situations with empathy andde-escalation techniques.

• HolisticCare: Work in synergy with a multidisciplinary team, including therapists,case managers, and medical professionals, to provide holistic care thataddresses individuals' physical, emotional, and social well-being. You can keepthis.

• Advocacyand Self-Determination: Champion individuals' rights andself-determination. Assist them in making informed decisions about their livesand choices.

Qualifications:

• 2Year College Degree, preferred.
• Previousexperience in direct care and community involvement is a plus.
• Ademonstrated passion for promoting inclusion and empowerment.
• Excellentcommunication skills for fostering meaningful community connections.
• Strongproblem-solving skills and adaptable mindset.
• Validdriver's license and reliable transportation.
• Less than 6 points on your Driver's License.
