A Path of Care Management Services, LLC is Hiring a RN Case Manager Near Coalgate, OK

Career Description Summary: The RN Case Manager makes the initial evaluation visit, regularly reevaluates the patient’s nursing needs, initiates the plan of care and necessary revisions, and furnishes those services requiring substantial and specialized nursing skill. This position initiates appropriate preventive and rehabilitative nursing procedures, prepares clinical and progress notes, coordinates services, informs the physician and other personnel of changes in the patient’s condition and needs and counsels the patient and family in meeting nursing and related needs. The RN Case Manager participates in in-service programs, and supervises and teaches other nursing personnel. Travel 100% of the time.
Essential Functions & Responsibilities:
  • Completes initial and ongoing comprehensive assessments of the patient’s needs, including OASIS assessments at appropriate points in time.
  • Initiates the plan of care and revises as necessary under the direction of the physician.
  • Provides those services and/or treatments requiring substantial and specialized nursing skills.
  • Assesses and educates the patient and family/caregiver regarding the disease process, self-care techniques, prevention strategies and nursing related needs.
  • Initiates appropriate preventive and rehabilitative nursing procedures as ordered.
  • Prepares clinical and progress notes.
  • Coordinates services and refers to other services as needed.
  • Informs the physician and other staff of changes in the patient’s condition and needs.
  • Evaluates the effectiveness and outcomes of care.
  • Supervises, delegates, and teaches duties and tasks to Licensed Practical/Vocational Nurses, Home Health aides and others providing services, as appropriate.
  • Plans for discharge from service.
  • Participates in in-service programs.
  • Re-evaluates the patient’s nursing needs on a regular basis.
  • Accepts and carries out physician orders both oral and written.
  • Assists physician in evaluating the patient’s level of function.
  • Practices infection control and universal precautions to ensure the safety of the patient.
  • Recognizes and reports any form of abuse taking appropriate action according to policy and federal/state laws.
  • Lives the PLUS Standards in the care of patients, daily tasks and interactions with others.
  • Maintains regular attendance and punctuality which are critical in order to complete the day to day tasks of this position.
Marginal Job Functions:
  • Works in the office as needed when patient census is low.
  • Participates in training activities.
  • Assists with orientation of new nurses as requested.
Education and Experience Requirements: Must be a graduate of an approved school of professional nursing. Must have an active RN License in the State of Oklahoma and one year of previous nursing experience. Home Health, Critical Care or Emergency Room experience preferred. CPR Certification required within 60 days from date of hire. Must have reliable transportation, proof of insurance and an active state driver’s license in good standing. Must be able to travel long distances and handle the stress of frequent scheduling changes.
Essential Physical Requirements: The RN Case Manager works alone in the patient's home and may need to assist patients with ambulation. The RN Case Manager must be able to lift, push, pull and carry up to fifty (50) pounds. This position will require lifting, pushing, pulling and carrying more than 20 pounds up to 70% of the time. This position requires lifting, pushing, pulling and carrying more than 30 pounds up to 25% of the time.
Working Conditions: Constant contact with families, visitors, patients and employees. Will work in patient homes, other healthcare facilities, and offices. This position travels in all weather conditions. There is potential exposure to infectious diseases including AIDS and Hepatitis B viruses. (See Physical Demands for more information).
