LPN, Field Nurse responsibilities and duties:

• Communicates with RNs and RN Case Managers to ensure continuity of care and clarifies information when in doubt or unclear

• Daily injections administers by required times daily or on weekends.

• Assists the registered nurse in carrying out the plan of Care

• Teaches the patient appropriate self-care

LPN, Field Nurse qualifications and skills:

• LPN license with 1-2 years nursing experience

• Current CPR license

• Valid drivers license along with reliable transportation and proof of insurance

• Must pass full background check
