Job Description Summary: Home Health Visits

1. Ability to perform initial home care patient visit and re-evaluate the patient's needs and progress on routine basis, OASIS exp for RN role.

2. Initiate the plan of care under doctor's orders and coordinate patient care.

3. Provides direct patient assessments and care in the patient’s home.

4. Notify the physician and clinical personnel of change in the patient’s condition.

5. Provide excellent patient centered care, help decrease re-hospitalizations rates.

6. Provide education to patients and family.

7. Completes accurate and timely documentation consistent with the plan of care, agency protocol, and State / Federal guidelines.

8. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient, communicates D/C or ROC plan with clinical manager.

9. MUST be reliable, professional, and responsible.

Job Types: Full-time, Part-time

Pay: $35.00 - $70.00 per hour
