Duties

The RN Home Telehealth (HT) Care Coordinator directs care for a panel of patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective. A HT RN Care Coordinator works closely with program staff, the primary care provider (or providers), other healthcare professionals and team members, other clinics, internal or external services and community agencies, and medical center leadership. He/she provides professional assessment, coordination and planning of multiple health care services; acts on behalf of the veteran to assure that necessary clinical services are received and that progress is being made. In addition, the HT RN Care Coordinator provides ongoing evaluation of care management services provided to Veterans.
The HT RN Care Coordinator is under the direct supervision of the Connected Care Program Manager. The individual is knowledgeable of HT business, clinical and technical operations. The incumbent will coordinate care for a panel of patients through-out the continuum of care for a variety of health conditions including, but not limited to: hypertension, congestive heart failure, chronic obstructive pulmonary disease, diabetes mellitus, post traumatic stress disorder, dementia, depression/anxiety, bipolar disorder, schizophrenia, and weight management.
Duties:

• The Registered Nurse (Home Telehealth) will be responsible for the following duties:
• As the clinical HT-Nurse and role model, the nurse stays current with advancements in nursing practice through seminar attendance, review of nursing publications, etc.
• HT-nurse assumes the responsibility to maintain patient safety, standards of care/practice and accreditation standards.
• working closely with a preceptor with a goal of developing skills and judgment to utilize the nursing process in real situations.
• The HT-nurse models adherence to Veteran Health Administration (VHA) policies, standard operating procedures (SOP) and directives.
• He/she provides ideas for process and program improvement / development and actively works toward achieving quality outcome measures.
• Delegates care in a safe, efficient, and cost-effective manner.
• Assists patients in identifying and securing appropriate services.
• Using the nursing process and evidence-based practice to collaborate with the Team (Patient, Primary Care Provider, RN Care Manager, Clinical Associate and Clerical Associate) and expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven care plan for life.
• Collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs.
• Providing patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals.
• Demonstrating leadership by serving as an advocate for patients, and a team leader with colleagues as she/he continues to enhance his/her own and the team's professional growth, development, and practice.
• All other duties dictated by patient care.
