Job Summary
We are seeking a Care Coordinator to join our team. In this role, you will work collaboratively with clients to determine their non-medical needs, develop the best course of action, and oversee their care plan alongside the Team Lead (RN), ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about healthcare practices.
Responsibilities
  • Collaborate with referral sources, clients, families, and Admin staff
  • Maintain Client Secured Files
  • Develop individualized care plans
  • Educate clients on their home care options
  • Create goals and monitor progress toward goals
  • Community Liaison
  • Active Communication with clients and families
Qualifications
  • Previous experience as a Care Coordinator or in a similar position is preferred
  • Certification as a medical assistant is preferred
  • Strong problem-solving and organizational skills
  • Ability to manage multiple projects or tasks and prioritize appropriately
  • Ability to work in fast-paced situations and make sound decisions quickly
  • Excellent interpersonal skills and high level of compassion
  • Strong verbal and written communication skills
