We are looking for a Licensed Vocational Nurse to join our team and provide home health services to our patients. The ideal candidate for this position will have experience in psychiatry, addiction medicine, and long-term care. The successful candidate will be responsible for providing direct patient care, administering medications, and performing assessments of the patient's condition. We are looking for an individual who is passionate about helping others and is dedicated to providing quality care.

Responsibilities:

• Provide medication management to individuals including but not limited to medication administration, treatment and monitoring of side effects, monitoring, and supportive services to increase compliance, etc. Inform psychiatrist, case manager, and mental health provider, as appropriate, of medication concerns/issues
• Coordinate ABD psychiatric services and appointments by working closely with psychiatrists and providing follow-up consultations to patients.
• Maintain medication logs and medication storage in compliance with safety standards and industry standards of care
• Provide patient care in a home health setting.
• Provide direct patient care in a psychiatric setting.
• Assist in the management of addiction medicine patients.
• Provide direct patient care in a long-term care setting.
• Assess patient's physical and mental health status.
• Document the patient's progress and treatment plans.
• Maintain accurate records of patient care.
• Provide care and supervision to patients (measure vital signs, injections, skin checks, medication and incontinence management, communication with health care professionals, responsible parties and administrators, diabetes control)
• Inspect MAR to make sure complete and correct at all times for all patients (order medications, communicate with pharmacy and RN when needed)
• Maintaining confidentiality of patients' files HIPAA and all applicable guidelines
• Perform other duties as assigned

Qualifications/Experience

• At least 18 years of age
• Must be willing to participate in the Company's pre-employment screening process, including drug screening, fingerprinting, sex offender registry database search, and background investigation
• Must submit to a Criminal Record Clearance, TB Clearance, have a current BLS or CPR card, First Aid Certificate, Required Health Screening
• Knowledge of infection control procedures, including proper hand washing techniques, contact with blood spills, and standard precautions
• Good interpersonal skills, with the ability to interact effectively at various social levels and across diverse cultures.
• Knowledge of or willingness to learn state and federal regulations governing nursing care
• Able to produce data for reports at any given time, outgoing basis, and a 'last minute request
• Bilingual in English/Spanish preferred

Job Type: Full-time

Pay: $31.00 - $36.00 per hour
