Title: Director of Patient Care Services (DPCS)
Department: Homecare & Hospice
FLSA Status: Exempt
Reports To: Administrator
Summary: The DPCS is responsible for the overall direction of home health and hospice clinical services. The DPCS establishes, implements and evaluates goals and objectives for the home health and hospice services, practices clinical staff management and development, and contributes to the planning, marketing community development, and compliance of the services.
Essential Functions:
Staff Management & Development
• Coordinates and oversees all direct and indirect patient services provided by clinical personnel and ensures Agency policies and procedures are being followed.
• Provides guidance and counseling to the clinical supervisors and clinical personnel for continuous improvement of home health care services in all aspects.
• Keeps staff up to date with current clinical information and practices. Practices staff development including orientation, in-service education and continuing education.
• Hires, evaluates, and terminates clinical personnel.
• Maintains and evaluates the orientation program for new clinical personnel. Responsible for orientation of new organization personnel, either directly or by delegating to another staff member.
• Evaluates the performance of case managers and clinical staff as needed, including annual evaluations. Evaluates the competency of clinical staff with periodic supervisory visits.
• Plans and implements in-service and continuing education programs to meet the education and training needs of organization personnel.
Quality Assurance
• Assists with the evaluation of organization performance via quality assurance, performance improvement program, productivity, and staff performance and competency reviews.
• Responsible for review/audits of clinical assessments and plan of care.
• Serves as a member and reports to the Professional Advisory Board Group.
• Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
Planning & Strategy
• Participates with the Administrator in the formulation of a business plan including strategic goals, resource planning, marketing plan, etc.
• Assists in the preparation and development of annual budget and organizational goals.
• Consults with the Administrator to determine a staffing pattern, which will meet Agency objectives and promote maximum utilization of field staff.
• Interprets operational indicators to detect census changes that could impact staffing levels, revenues or expenses.
Marketing & Community Development
• Assists Administrator in evaluating community needs, and plans programs and services to meet the identified needs, including appropriate staff and program development.
• Assists in marketing the Agency and fostering community relations.
Regulatory Compliance
• Adheres to all Medicare regulations related to conditions of participation and State regulations, and scopes of practice, and maintains compliance with the CHAP Home Care and Hospice standards.
• Maintains and updates policy and procedures manuals. Ensures that the Agency operates in compliance with legal and regulatory standards.
• Works with the Administrator to ensure compliance with contracted health plans and prepare for evaluations and audits to maintain credential status.
• Assures proper maintenance of patient clinical records, statistics, reports and records in compliance with local, State and Federal laws.
• Intakes coordination, including oversight of intake process and communication between disciplines, patients and physicians, maintains and improves clinical scheduling process, maintains MD order tracking system, and facilitates DME and supply requests.
• Assures 24-hour clinical coverage for hospice patients.
Other Job Functions/Responsibilities
• Serves as a clinical resource person to other Self-Help for the Elderly health care programs and services, including Home Health Aide Training Program and Palliative Care Services.
• In the absence of the Administrator, the DPCS will become the acting Administrator and will be vested with the authority to act on behalf of the Administrator.
• Other duties as assigned by the Administrator or government board.
Qualifications:
• Must be a current California Licensed Registered Nurse with at least three years of experience in a home health agency, primary care clinic or health facility (within the past five years).
• Minimum one year of supervisory experience in a home health agency, primary care clinic, or health facility.
• Knowledge of relevant federal and state regulations, and guidelines for current home health assessments and care classifications.
• Must have excellent observation, verbal and written communication skills.
• Must be a passionate self-starter that thrives in a team environment. Must have strong critical thinking and problem-solving skills and must have excellent attention to detail with an eye on the big picture.
• Must have excellent interpersonal and conflict resolution, mediation, and crisis management skills, with the ability to exercise good judgment and confidentiality.
• Ability to maintain comprehensive working knowledge regarding information systems and applicable software programs like electronic medical records.
• Must be available for on-call duty outside of office hours per arranged schedule.
• Must pass health screening for TB clearance, physical examination, and background check.
• Must be able to travel via public transportation or personal/department vehicle for the performance of job duties. If driving must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
