Description

1736 Family Crisis Center offers crisis intervention services, including four 24- hour hotlines, as well as longer-term services in client advocacy and outpatient psychotherapy. The residential counseling and support services include shelter programs for (1) runaway and homeless adolescents, (2) domestic violence and human trafficking victims and their children (emergency and transitional programs (3) Homeless prevention and rapid re-housing services for the general population and Veterans and their families. Community outreach programs are provided to educate the public and prevent mental-health problems in the general population and in targeted groups to at-risk populations.

We are currently seeking a full-time Case Manager/Job Developer for our new Bridge Home Project. This is an opportunity for an experienced case manager with a passion for community service and advocacy. This project will provide job development skills and case management to a 15-bed facility for women experiencing homelessness. Hours may vary and are based upon the needs of the residents.

Job Responsibilities:

  • Conduct assessment and intake screening of all clients, enter program data into the region’s Homeless Management Information System (HMIS) and/or established computer databases to document client activities and services. Maintaining highly organized files for each client.
  • Assess individuals as appropriate, utilizing the Vulnerability Index (VI) SPDAT by establishing successful rapport with clients using trauma informed skills.
  • Work with clients to provide case management services, including assessment of client’s housing needs, and link clients to care coordination services including other community resources such as physical health services, mental health, and other supportive services. Assist clients to enroll in mainstream benefits and obtain identification (i.e. California Driver’s License, Social Security Card).
  • Actively research potential job opportunities. Through the use of outreach strategies effectively develop and maintain relationships with prospective employers throughout the community. Provide soft skills/ training to clients in both group and one-on-one settings, within areas such as interview skills, attire and professional etiquette.
  • Preparation, coordination and service of meals for site.
  • Ensure that shelter is accessed only by permitted parties only by authorizing the entrance and exit of deemed clients, employees, vendors and other authorized individuals by participating in regular patrol of site, ensure safe practices and guideline adherence by clients
  • Ensure accurate recording of client daily sign-in/out log and address any bed night issues with clients.
  • Conduct bag/belongings search and screening for each individual participant enrolling in program prior to entry and upon regular daily return to site for every client. Enforce guidelines regarding items allowed on site for safety purposes
  • Conduct scheduled room checks for adherence to safety/cleanliness standards, monitor and report any safety concerns in relation to residential program
  • Ensure safety of site by reporting any maintenance needs and physical site issues
  • Provide Crisis intervention, client de-escalation and notify proper authorities as needed in case of emergency. Lead site evacuation and/or ensure other safety protocols are followed
  • Facilitate house meetings and any other site activities requiring coordination
  • Participate in case conferencing meetings and other community meetings.
  • All additional duties as assigned, as this is not a comprehensive list of duties, shifts and schedules not guaranteed are determined by the needs of the residents

Qualifications


Qualifications:

  • Bachelor’s Degree from an accredited college/university in mental health, social services or comparable field and 1-year experience with homeless population, particularly issues related to women experiencing homelessness, in a residential or related living facility and related culture (combination of education and experience will be considered).
  • Demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First, including knowledge of issues related to women's poverty, homelessness, mental illness, trauma, substance abuse, health issues.
  • Strong knowledge of boundaries, and client focused care, HIPAA, programmatic standards of residential operations.
  • Familiarity with the Coordinated Entry System, Clarity HMIS and Interim Housing program data requirements.
  • Demonstrated ability to clearly communicate verbally and demonstrate professional writing skills.
  • Knowledge of residential site security issues and ability to enforce all guidelines related to safety.
  • Knowledge and experience with Service/Emotional Support Animals.
