Responsibilities:
Patient Care Coordinator will be responsible for ensuring a smooth and efficient intake process for clients seeking our services
They will be the first point of contact for consumers, providing support and guidance throughout the intake procedure
The ideal candidate thrives in a fast-paced environment and possesses excellent communication and customer service skills
Greet and welcome clients seeking services, exhibiting empathy and understanding towards their needs
Conduct thorough intake assessments to gather necessary information, including medical history, personal information, and support needs
Provide guidance and information to clients regarding available services, eligibility criteria, and financial assistance options
Assist clients in completing required paperwork and obtaining necessary documentation
Ensure all documents are accurately completed and maintained in an organized manner
Coordinate and schedule deliveries for clients, ensuring timely access to required products and services
Work closely with case specialist and case managers in order to provide exceptional services toconsumers
Follow up with consumers after product or services is delivered addressing any concern or questions they have
Participate in training programs and professional development opportunities to enhance knowledge and skills
Must have experience in healthcare
Job Types: Full-time, Part-time
Pay: $16.00 - $18.00 per hour
