10 BCFS Health and Human Services is Hiring a Remote Our House Case Manager

Education: Required- 4-year bachelor or undergraduate degree from an accredited university, in one of the following areas: Social Work, Sociology, Psychology, Counseling, or Criminal Justice. Experience: Required- 2 years of experience providing direct case management services. Preferred Master’s degree in the above listed fields and supervisory experience. Supervises: Resident Advisor, mentors and interns Summary: The Case Manager is responsible for using sound professional judgement and best practices to provide intensive case management to the residents of Our House including crisis intervention as needed while maintaining compliance with established state and federal standards. The case manager will lead the charge of successfully transitioning youth experiencing homelessness into adulthood through the promotion of self-sufficient living. Essential Functions: 1.Maintain a caseload, providing daily support and intensive home-based intervention case management services using a Trauma-Informed Care approach and evidence informed strategies to attain sustainable living by providing service planning and documentation of services for assigned caseload of young adult residents in the Our House program. 2.Ensure the home is always in excellent working condition and safe for residents. Report any damage, unmet needs, and/or maintenance matters up the chain of command and take immediate action to remediate the problem. 3. Evaluates the home environment on a weekly basis including monitoring that each resident always has their basic needs met. 4. Provide crises intervention responding 24/7 promptly and professionally to crises situations with residents following established chain of command reporting protocol. 5. Provide linkages to community resources for residents to assist with enrollment of additional programming and community support services that are identified in assessments, in interviews/meetings with residents, in ongoing treatment planning, etc. 6. Provide opportunities for growth in spiritual and cultural awareness areas per contract requirements and CSD Cultural Diversity plan. 7. Identify volunteers, complete the required background check process, promote their involvement and support with the residents, plan for volunteer opportunities including engaging the residents in community services activities to develop their social network and positive involvement in their community. 8. Lead group activities – life skills, outings, educational opportunities. 9. Identify homeless young adults who are eligible for support services although not residing at Our House, and ensure they receive services as outlined in the contract. 10. Ongoing outreach and community education on the homeless population, services that are available, and to identify eligible participants for the Our House program. This includes being a member of local coalitions, participating in community partner meetings, creating flyers and informational pamphlets for distribution, updating social media and website in order to support, identify and reach this population. 11. Mandated reporter - Report any suspected abuse or neglect to chain of command and all appropriate state and county enforcement agencies immediately and per policy and procedures. 12. Attend and participate in staff meetings, in-service trainings, weekly consultation with supervisor, planning meetings, and meetings with contract personnel as requested. 13. Maintain confidentiality in all areas of the service population and program operations. 14. Maintain BCFS HHS professional and ethical standards of conduct outlined in the BCFS employee policy manual including demonstrating appropriate boundaries with residents, respect for agency staff, the service population, and community members and complying with the required dress code at all times. 15. Work flexible hours as needed including evenings, weekends and holidays to resolve crisis situations when they arise. Measurable Deliverables: 1. Meet all federal and state regulatory guidelines and standards that are applicable to this position 100% of the time. 2. Maintain a full caseload 100% of the time in accordance with agency policies and procedures and contract standards. 3. Update and maintain a community resource list on a monthly basis, available to residents and other BCFS HHS programs. 4. Complete monthly schedules for weekly all resident meetings that are developed based on resident feedback, current need, and relevant to what residents are experiencing. These are completed the month prior and once approved by chain of command this position will ensure they are planned, successfully implemented, and documented in resident files. 5. Ensure each resident has their basic needs met 100% of the time while in the Our House program. 6. Complete and document room checks and walk-throughs of the home on a weekly basis. 7. Provide services to youth enrolled in the Transitional Housing Program including weekly one on one case management support that reviews and updates the individual treatment plan creating goals and making changes/updates as needed. 8. Provide weekly consultation and ongoing support and training to the resident advisor, mentors, and interns. 9. Create and maintain mentor program by engaging community volunteers, providing ongoing trainings, providing opportunities for residents and mentors to engage with one another, and providing ongoing individual support as needed. Ensure each resident is assigned a mentor within the first month of placement in the Our House program. 10. Seek out training opportunities for own staff development as required and outlined in the contract. Maintain contract compliance by completing these trainings as required. Utilize skills learned in training and documents use of learned techniques/evidence-based practices. 11. Conduct monthly community outreach in order to meet the deliverables for the program. 12. Conduct and document follow up calls and/or visits to young adults in crises situations and complete safety planning as needed. 13. Immediately upon placement into the home and per procedures, work with the resident to develop, monitor, and review Individual Treatment Plans designed to increase strengths, address needs, and reduce conflict in the home. Use these plans to guide case management services. 14. Compile, prepare, submit and maintain accurate case records, files, forms and additional information in accordance with agency policies, and/or contract requirements. This includes all documentation recorded within 24 business hours. 15. Complete all required paperwork, including youth assessments, and analyze the results of each assessment to best develop a plan, and present professional decision on eligibility and acceptance into the program to chain of command, prior to youth acceptance into the Transitional Housing Program. 16. Begin exit planning immediately upon placement into the program and locate appropriate housing for clients prior to their last day in the Transitional Housing program to ensure smoot transition to independent living. 17. Implement BCFS HHS safety protocols per the Our House handbook, policies and procedures, CSD disaster emergency response plan which includes need for monthly evacuation drills, creating exit plans for the building, and providing residents with informational signage to include, but not limited to medical services available in their community, how to notify BCFS HHS personnel in an emergency, visitors in the home, etc... 18. Update tracking boards to ensure resident information is readily available in case of an emergency. 19. Submit written incident reports immediately following an incident and after sharing information up the chain of command. Credentials, Skills and Abilities: 1. Maintain current licenses and certificates required for the position including but not limited to:  First Aid  CPR  Other ________________________________________ 2. Pass a pre-employment drug screen, random drug screens and maintain emotional control and professional composure at all times. 3. Provide proof or work eligibility status upon request. 4. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years) 5. Demonstrate the ability to: a. Respond sensitively and competently to the service population’s cultural and socio-economic characteristics. b. Communicate effectively in writing and verbally. c. Work in a fast-paced environment. d. Maintain computer literacy required to meet the responsibilities of the position. e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team. f. Travel as needed (community travel for outreach and meetings; out of town travel for annual trainings and support from headquarters). g. Manage time effectively and efficiently. h. Utilize excellent organizational skills to plan and implement training, meetings and structure for the residents of the program.
