Education: Bachelor’s degree in a Health and Human Services, social work, and social service field or currently enrolled in an advanced degree program. Education may be substituted with at least 5 years of experience in the field of Health and Human Services. Experience: Required 3 years related direct experience serving families. Supervises: None Summary: The Home Visitor is responsible for using sound professional judgment and best practices to ensure quality services are provided to the service population while maintaining compliance with established program standards. To successfully perform this job, the employee may be expected to perform the following, with or without reasonable accommodations in accordance with the Americans with Disabilities Act of 1990: Essential Functions: 1. Outreach and recruitment of participants for the Texas Home Visiting Program. 2. Establish and maintain a trusting and professional working relationship with families by providing regular contact. 3. Utilize the Safe care curriculum with caregivers, which is evidence-based. HV will also report to the Program Director to be coached on required Safe care recordings with caregivers. 4. Administer intake, assessments, and screening tools to clients and family members. 5. Utilize intake and assessment results to establish and effectively implement goals, objectives, and activities identified with parents. 6. Provide intensive case management services to families. 7. Provide parent health education, case management, resource and referrals, basic needs support, transportation assistance, and/or other support services to families on caseload 8. Conduct systematic follow-ups for all families. 9. Create and set-up required events on monthly calendar. 10. Record all group and individualized services and activities in a complete, concise, and timely manner utilizing program forms into PEIRS (DFPS), Salesforce, and (BCFS) software. 11. Meet with supervisor on a bi-weekly basis to evaluate status of families on caseload. 12. Meet all deliverables as outlined in contract. Measurable Deliverables: 1. Obtains and maintains certification in Safe Care curriculum administration per National Safe Care Research and Training Center guidelines. 2. Document all attendance on a weekly basis in Salesforce. 3. Attend all required annual trainings. 4. Provide community education on child abuse prevention strategies monthly. 5. Develop together with Lead Resource Navigator, updates, reminders, and community education information monthly. Credentials, Skills and Abilities: 1. Maintain current licenses and certificates required for the position including but not limited to: • First Aid • CPR 2. Pass a pre-employment drug screen and random drug screens and maintain emotional control, and professional composure at all times. 3. Provide proof or work eligibility status upon request. 4. Pass a pre-employment and biennial criminal background and motor vehicle check 5. Demonstrate the ability to: a. Respond sensitively and competently to the service population’s cultural and socio-economic characteristics. b. Communicate effectively in writing and verbally in English and Spanish. c. Work in a fast-paced environment. d. Maintain computer literacy required to meet the responsibilities of the position. e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team. f. Travel as needed. g. Manage time effectively and efficiently. 6.
