Your RoleAs the Service Facilitator for Sertoma’s Intermittent Community Integrated Living Arrangement (I-CILA), you will provide care and support to the consumers within the Intermittent Residential Program. This includes the areas of daily living, skill development, community integration, and medication management. This position is responsible for the day-to-day implementation and facilitation of services to individuals residing in the ICILA homes they are assigned to, ensuring services are delivered in a person-centered manner and is a member of the Community Support Team for each individual residing in the ICILA home. This position requires some flexibility in order to support consumers in the ICILA program (i.e. days, evenings, and weekends as needed).
Responsibilities
  • Responsible to ensure performance of job responsibilities in compliance with DHS Rule 115 & 116, CARF standards, agency mission, agency policy & procedure, program guidelines and best practice. Access manuals located in the home or from their supervisor for clarification regarding policy, guidelines and licensing standards.
  • Responsible for implementation of ISP and intervention plans including to provide the level of supervision outlined in the plan. Responsible and accountable for the location and activities of all individuals in the home. Utilize natural environment for training opportunities including activities of daily living. Provide the level of assistance requested or needed to individuals in completion of daily tasks/responsibilities/activities.
  • Attend, participate, and successfully complete all required DHS training and general training within Sertoma Centre guidelines. Ensure that day-to-day job performance reflects/demonstrates competency in all areas trained. Attend all mandatory staff meetings providing written input if excused from attending by supervisor.
  • Communicate in a professional, respectful and effective manner promoting an atmosphere that is responsive to the needs and desires of individuals served and reflects a commitment to the delivery of services in a person centered manner. Provide accurate information regarding individuals served on a “need-to-know” basis. Communicate concerns from individuals served and/or their families in a timely and effective manner so that issues may be addressed. Ensure the confidentiality of information regarding individuals served.
  • Complete and submit all required documentation by assigned deadlines including, but not limited to, service notes, intervention plans, ISPs, incident & accident reports, goal and behavior tracking, medication documentation, emergency drills, physical plant reviews, temperature logs, safety checklist, and housekeeping documentation as required. Ensure that documentation is accurate and utilize appropriate grammar, spelling and terminology.
  • Ensure a safe and healthy environment that fosters independence and self-determination. Ensure that all housekeeping tasks are completed, that program items, supplies, medication and food are stored and used as per guidelines, that all safety issues are immediately brought to the attention of the immediate supervisor, and provides levels of supervision appropriate to the activity.
  • Take direction from the individuals served, CILA Operations Manager, Residential Service Coordinator, and, the Director of Residential Services, to ensure that the needs and priorities of those served is effectively addressed.
Qualifications
  • High School Diploma or GED recognized by the Illinois Department of Human Services required.
  • Demonstrated 8th grade reading level as evidenced by standardized testing recognized by the Illinois Department of Human Services.
  • Valid Illinois driver's license with good driving record in keeping with agency driver qualifications guidelines.
  • Completion of all required DSP training within 120 days of hire and maintenance of required certifications.
  • Minimum of one year experience working directly with individuals with developmental disabilities preferred.
