Description of Position:

The Licensed Practical Nurse (LPN) ensures the general well-being of residents by providing a system of services for residents in a manner and environment that provides dignity and full recognition of each resident’s individuality. The Licensed Practical Nurse will manage and supervise the delivery of services by Caregivers and CNAs. The Licensed Practical Nurse respectfully interacts with all residents, family members, visitors, colleagues, and staff. This is a safety sensitive position.

Responsibilities of the Licensed Practical Nurse:

1. Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
2. Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.
3. Ensure that all nursing service personnel comply with the procedures set forth in the Nursing Service Procedures Manual.
4. Make written and oral reports/recommendations concerning the activities of your shift as required.
5. Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to the care.
6. Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Nurse Supervisor.
7. Fill out and complete transfer forms in accordance with established procedures.
8. Ensure that appropriate documentation concerning unauthorized discharges is entered in the resident's medical record in accordance with established procedures.
9. Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
10. Sign and date all entries made in the resident's medical record.
