Position Summary:

Responsible for providing primary care in a family-based clinic environment within an interdisciplinary team to achieve HealthPoint mission and goals in a holistic manner including the assessment of physical, psychosocial, health education, and discharge needs of for the patients we serve while maintaining a high level of professionalism and quality care across the life span in a clinic, virtual or community/shelter settings.

Compensation is dependent on skills and experience.

Your contribution to the team includes:

  • Use client data and nursing scientific principles to develop nursing diagnosis and to identify client problems in order to deliver effective nursing care.
  • Implements the plan of care by initiating nursing interventions through giving direct care and supervising other members of the care team.
  • Collaborates with patient and Care Team for wrap around services.
  • Performs independent visits while operating under standing order procedures.
  • Triage of patient phone calls for acuteness of need and/or priority of intervention and address findings within the interdisciplinary care team.
  • Assess walk-in patients for acuteness of need and/or priority of intervention and document assessment information of all activities.
  • Education of patients, families, and communities in a wide variety of health care subjects such as maternal/child health care, nutrition, hypertension, diabetes and lifestyle, sexually transmitted infections, basic health care maintenance, and self-administration of medications with chronic and acute conditions.
  • Perform complex procedures as appropriate for the RN scope of licensure and the individual RN's competency (may include wound care, IV insertion and fluid/medication administration, urinary catheter insertion and removal).
  • Perform patient follow-up including notifying patients of laboratory results, scheduling referral appointments, maintaining charts and records and responding to patient telephone calls.
  • Monitoring patients' ongoing conditions, adjusting treatment plans, and health promotion as needed.
  • Provide motivational interviewing and harm reduction counseling.
  • Utilize provided reports to identify high-risk patients and high volume/high-cost services; track and improve utilization over time.
  • Lead, engage, participate improving the performance of identified quality measures and population health initiatives.
  • Engage and collaborate in case management related to specific patient populations to ensure utilization of specialized healthcare services.
  • Execute cost containment strategies around medication reconciliation, discharge follow-up phone calls and pre- and post-visit care coordination with patient in a clinic, virtual or community/shelter setting.
  • Use provided reports to identify high-risk patients and high volume/high-cost services; track and improve utilization over time.
  • Lead, engage, facilitate and support in a preceptorship model students and new nurses in the onboarding and training process.
  • Provide support and education of back-office staff and colleagues that includes mentoring, training and oversight in basic skills and procedures, testing knowledge base and giving in-services.
  • The float Nurse Clinician III will work throughout various HealthPoint locations to provide coverage for shortages as well as providing additional support with other assigned duties. Responsible for monitoring and staying current on daily schedule. Must be flexible and able to adapt to multiple clinic environments and patient populations while maintaining a high level of professionalism and quality patient care.

Must have's you'll need to be successful:

  • Minimum BSN five (5) years of experience or 15 years equivalent and advanced degree or certification required.
  • Degree in Nursing from an accredited college or university.
  • Unencumbered Washington State Registered Nurse license required.
  • Current BLS certification
  • National Certification in Ambulatory Care or similar type certification desired.

Proof of vaccination for COVID-19 is required, prior to start.All new employees are also required to show proof of immunizations and/or immunity to MMR (measles, mumps, rubella), Varicella, annual Influenza and TB QuantiFERON Gold Titer. Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to blood/blood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immune titer results prior to starting or no later than their fifth (5) business day of employment.
