Wages & Benefits

$­­­­25.00 - $28.85 per hour; company-paid medical, dental, vision, life insurance, AD&D, and short-term and long-term disability; health savings account; company-sponsored retirement plan with automatic enrollment, a 10% employer contribution after 1 year of service, immediate vesting, and 100% employer matching on the first 5% of employee contributions and paid parental leave for eligible employees.

Schedule

This position is in-person and located in an office building that is accessible and smoke free. This position works during normal business hours and may include occasional evening and/or weekend hours.

A Typical Day in the Life…

A typical day as Hallie Q. Brown’s Community Support Navigator provides an opportunity to work closely with other HQB staff to bridge connections to community resources and leverage the strengths and expertise of our program participants. Responsibilities will include the following:

  • Providing a safe, welcoming environment for all clients and visitors.
  • Connecting with current HQB clients regarding resource needs.
  • Conducting intakes, including gathering physical and social information to determine the clients’ needs.
  • Determining client eligibility for government programs.
  • Developing individual service and support plans and monitoring outcomes.
  • Coordinating care for clients, including advocating with external providers.
  • Conducting periodic follow-up on clients who have exited case management.
  • Responding to crisis situations in a professional manner and providing follow-up with clients as needed.
  • Maintaining a broad list of providers and community resources available to clients.
  • Building and maintaining collaborative relationships with a range of providers.
  • Promoting and representing HQB at external events as requested.
  • Assisting other employees and departments as needed.
  • Actively and positively contributing to HQB’s mission and work environment.

Who You Are

  • You hold a bachelor’s or master’s degree in social work or a closely related field, or you have demonstrably equivalent knowledge gained through alternative courses of study and life experience.
  • You have a minimum of three years of professional experience working with diverse populations that face barriers to success.
  • You have previous experience completing intakes, assessments, creating goal plans, and tracking outcomes.
  • You have previous experience helping others navigate complex systems.
  • You have a demonstrated interest in the human services nonprofit sector; experience working in a nonprofit setting is preferred.
  • Previous knowledge of the Rondo area is desired.
  • You have the ability to foster an inclusive, safe, and productive environment.
  • You demonstrate an understanding of general, public, and federal assistance programs and other community resources.
  • You have excellent verbal and written communication skills, especially for client advocacy.
  • You have a demonstrated ability to efficiently organize, coordinate, track, and complete multiple tasks and adjust to changing priorities.
  • You have the ability to exhibit appropriate care and concern for clients within the context of a professional relationship.
  • You demonstrate a high level of personal integrity with the ability to maintain confidentiality.
  • You demonstrate a willingness to learn a variety of tasks, a serious work ethic, and a desire to work in a team-orientated environment.
  • You have the ability to work independently, establish priorities, and meet deadlines.
  • You demonstrate an understanding of the importance of customer service and being professional.
  • You have knowledge of and previous experience working with diverse groups of people of lower socioeconomic conditions.
  • You are skilled in conflict management.
  • You are competent in MS Office: Word, Access, Publisher, Outlook, Excel, and PowerPoint.
  • You have a valid driver’s license, access to reliable transportation, and the ability to meet Hallie Q. Brown’s driving record requirements. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit (not applicable for new hires living out of state).
