Rural Health Group, Inc. is Hiring a High Risk Referral Specialist (Clinical) - CMA/LPN/RN Near Roanoke Rapids, NC
Rural Health Group, a multi-location Community Health Center in northeastern North Carolina, is seeking a CMA, LPN, or RN for full time position as a High Risk Referral Specialist to support our clinical team at RHG's clinic at Halifax Medical Specialists . This role will work Monday - Friday 8 am to 5 pm. The Clinical Assistant facilitates services aimed at enhancing patient-centered care and maximizing outcomes across the patient care continuum. The duties of the position include patient advocacy, monitoring patient care to ensure progress toward desired outcome, addressing patient and family needs, resolving obstacles to effective care, and together with the clinical team, planning for patient treatment and follow-up care. Must be Certified MA or have an active North Carolina LPN or RN license. CNA applicants or MAs who are not yet certified will not be considered. The following clinical skills/experience are preferred:
Phlebotomy
Equipment usage (EKG, clinitek, Hemoglobin)
Good documentation skills
Tracking and maintaining medical data
Immunization (adult and pediatric)
Injections
IV therapy
Strong understanding of disease management for providing patient education at the direction of provider
Active North Carolina LPN or RN license, or MA Certification/Registration
Expanded nurse role experience preferred
Excellent organizational, telephone and communication skills
Excellent leadership and teamwork skills
High school diploma or GED
Ability to juggle multiple priorities and work efficiently in a potentially stressful environment
Ability to apply good judgment while carrying out responsibilities
Pleasant, professional approach; must be able to work well with the public and provide excellent internal and external customer service
Ability to establish and maintain positive, effective, professional relationships with patients, providers, coworkers, and superiors
Possess knowledge of modern office equipment and Microsoft Office software programs
Basic computer skills; must be able to quickly learn to use eClinicalWorks electronic health record system
Bilingual (English/Spanish) is a plus, but not required; applicants who may serve in a translating capacity will be required to take a language assessment
