About the Opportunity
The Director of Quality Improvement is a professional responsible for analyzing data integrity and consistency of OASIS documentation and assessment processes. This position will ensure appropriate ICD-10 coding and sequencing and will work with clinical staff to clarify documentation and data integrity issues.
Duties and Responsibilities
  • Prospectively reviews all OASIS assessments as assigned to ensure appropriateness, completeness, and compliance with federal and state regulations.
  • Utilizes OASIS variation or alert reports when reviewing OASIS data.
  • Ensures appropriate ICD-10 coding and sequencing as it relates to the patient's medical condition, including any co-morbidities.
  • Consults with appropriate clinical staff to clarify any data integrity issues and works with clinician to make appropriate corrections
  • Reviews visit utilization for appropriateness of care guidelines and patient condition; reports potential financial losses and/or underutilization to the clinical manager/designee.
  • Analyzes problematic trends as a result of OASIS review and makes recommendations to leadership team.
  • Works with managers to address trends that affect the agency's outcome and process measures noted during OASIS review.
  • Makes OASIS inter-rater reliability visits with clinical staff.
  • Participates in Quality Improvement and Corporate Compliance activities as assigned.
  • Assists with other chart audit activities as assigned.
    • Coordinates activities related to clinical record review for data collection.
    • Compiles, trends and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes, and customer satisfaction. Data is analyzed and compared internally and externally with other sources when available.
    • Works collaboratively with nursing supervisors to ensure documentation is complete and consistent with care and reflects legal requirements.
  • Manages QAPI program.
  • Collaborates with Clinical Managers to provide a process to promote and ensure employee orientation, competency and in-service education programs.
    • Assists in the identification of skills for annual competence and provides in-service training support.
  • Maintains current knowledge related to Medicare Conditions of Participation and the Joint Commission's standards to ensure compliant with state, federal and accreditation guidelines.
  • Responsible for ensuring policies and procedures as related to OASIS data collection reflect all related local, state, federal and Joint Commission standards and makes recommendation for policy changes to the governing board.
  • Acts as a resource to the Director of Patient Care Services and Clinical Managers and the regarding standards, policies, regulations and provides in-service training to leadership and personnel as needed in these areas.
  • Acts as a clinical resource to personnel.
    • Maintains updated therapy knowledge and skills.
    • Assists with patient visits as needed.
    • Promotes self-growth and collegial relationships with others in the home care industry.
    • Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal and professional networks and participates in professional networks or societies.

Qualifications
Minimum Requirements
  • Registered Professional with current licensure to practice in the State.
  • Excellent understanding of performance improvement, quality assurance, and utilization management.
  • Detailed oriented, self-directed and able to work with minimal supervision.
  • Have excellent coordination and communication skills.
  • One (1) to two (2) years of clinical home health experience required.
  • Working knowledge of OASIS and ICD-10 coding. OASIS certification (COS-C) preferred; OASIS Home health ICD-10 coding certification preferred.
  • Knowledge of federal regulations and state licensure requirements.
  • Working knowledge of computer systems.
  • Excellent coordination and communication skills.
