Primary Care of WNY is one of the largest Family Medicine practices in Western New York. We deliver care that places the patient at the center of everything we do. Our five offices are conveniently located across WNY including: Williamsville, Kenmore, Lancaster, and Wheatfield.
As a Care RN you will provide ongoing support to our medically complex patients by completing assessments, developing care plans and coordinate care for the patient. You will also track hospital admissions and ER visits and provide follow-up.
Primary Areas of Responsibilities:
1. Ensure a high level of patient satisfaction
2. Maintain ongoing communication, support and education with patients in specific populations
3. Track hospitalized patients and assure transition of care upon discharge
4. Assist practitioners in complying the quality of care measures for their patients
5. Coordinate patient care ensuring that the providers plan of care is carried out within NYS RN scope of practice
6. Knowledgeable of care management principles and proven proficiency in nursing care management
7. Administers vaccines in accordance with pediatric and adult guidelines and practitioner orders.
