MetroPlusHealth is Hiring a Care Management Associate - MLTC Near New York, NY

The primary goal of the Personal Health Coach is to optimize members’ health care and delivery of care experience with expected cost savings due to improved quality of care. This is accomplished through engagement and understanding of the member’s needs, environment, providers, support system and optimization of services available to them. Under the direction of the Team Leader, the Personal Health Coach works to coordinate services using all available internal and external community resources. As an integral team member, the Personal Health Coach will work in conjunction with the Care Manager to ensure the member receives necessary support, quality care, connection to providers and community resources, and support members’ well-being to ensure quality outcomes (i.e., reduction in emergency room visits and hospital admissions, improved member satisfaction, closing and reducing Gaps in Care) and cost-effectiveness.

  • Problem solves member’s issues and needs: clinical, psychosocial, financial, and environmental.
  • Perform non-clinical activities for members of varying age, clinical scenario, culture, financial means, social support, and motivation.
  • Screen members according to MetroPlusHealth policies and procedures, assisting members to identify appropriate support services to maximize health and wellness.
  • Conducts telephonic outreach to members as directed by the Care Managers or Team Leader.
  • Troubleshoots member issues related to transportation, appointments, DME, medication, SDoH
  • Ensure members have appointments with their providers.
  • Engage members/caregivers in a collaborative relationship, empowering them to manage their physical, psychosocial, and environmental health to improve and maintain lifelong well-being.
  • Coordinate required services by members, caregivers, clinical and non–clinical providers, and care managers when involved.
  • Maximize member’s access to available resources.
  • Promote communication, both internally and externally to enhance effectiveness of clinical and non-clinical services and overall case management services.
  • Assist members in understanding mechanics and management of health care and social support networks.
  • Provide care coordination including but not limited to; ensuring access to care, reducing unnecessary hospitalizations, and appropriately referring to community supports.
  • Protect the confidentiality of member information and adheres to all company policies regarding confidentiality.
  • Consult with Care Manager and Team Leader regarding clinical status and treatment options for members.
  • Maintain accurate, comprehensive documentation of required information in compliance with risk management, regulatory and accreditation requirements.
  • Collect and prepare data, statistics, proposals, and reports relevant to responsibility as a Personal Health Coach.
  • Comply with all orientation requirements, annual and other mandatory trainings, organizational and departmental policies, and procedures, and actively participate in the evaluation process.
  • Participate in case conferences and departmental meetings.
  • Protect the confidentiality of member information and adheres to all company policies regarding confidentiality.
  • Bachelor’s degree in a health-related field required.
  • Two or more years of experience in a health care setting.
  • Prior experience in Care Management and/or Managed Care setting is strongly preferred.
  • Proficiency with computers navigating multiple systems and web-based applications.
  • Must know how to use Microsoft Office applications including Word, Excel, PowerPoint, and Outlook.
