Overview

Under the supervision or a registered nurse or the Center manager, provides care to participants of the Adult Day Center. The LPN's practice reflects the laws and regulations for Licensed Practical Nurse as outlined by the Commonwealth of Virginia.

Responsibilities

Under the supervision of the manager and/or registered nurse, utilizes the nursing process as a frame of reference for the provision of patient care within the scope of nursing practice Collaborates with the interdisciplinary team to plan participant care Serves as liaison with medical providers, care givers, family members to ensure appropriate and timely medical treatments for the participant Provides ongoing assessment and documentation of participants' health and care needs. Thinks critically as evidenced by appropriate clinical decision-making and prioritization. Creates and maintains an environment that protects participants from harm. Ensures appropriate participant and/or caregiver education with utilization of applicable resources Handles conflict resolution opportunities in an appropriate, timely, and consistent manner. Reviews participant charting in order to ensure optimal patient care and compliance with licensure. Evaluates effectiveness of care provided by the Center staff by means of record reviews and direct observation, educating staff on best practices. Uses resources (e.g., time, people, supplies, etc.) cost effectively. Other duties as assigned

Qualifications

Required Education: Graduate of an accredited Practical Nursing Program Preferred Education: Required Experience: Preferred Experience: Required Certifications and Licensures: Hold a current, active license as a practical nurse in Virginia or hold a current multistate licensure privilege as a practical nurse.
