Description

Job Summary: 

The Resource and Referral Coordinator – (RRC) receives inquiries and requests for information and services for young children and their families and provides direction and assistance to the referring party and family in linking to needed/requested services. The RRC provides family support, resource coordination, and parent education as needed. In addition, the position will provide support and relief for the Front Desk of the main EPU center.

Essential Functions:

  • Answer calls, obtain and document information (including grant specific data and information) into database system from referring party and parent regarding reason for referral including but not limited to 12 counties covered under Parent Education and Support catchment counties and their grants. 
  • Provide relief and support for the front desk at the main EPU office.
  • Contact all referrals received from CVRC for 0-3, ensure accuracy of information, upon conformation enter family referral into system as appropriate based on establish protocol within 24 hours of receipt. 
  • Make appropriate referrals based on needs related to developmentally, linguistically, and culturally appropriate services. 
  • Maintain a current listing of resources available to infants and children in 12 catchment counties primarily Fresno, Madera, Merced, Kern, Kings, Tulare, and Mariposa County.
  • Participate in Center multidisciplinary teams as appropriate and participate in team building activities.
  • Accurately complete and submit all necessary data collection and program documentation, including time studies daily, submit time study referral log to Director daily.
  • Provide assistance to other members of the team as needed.
  • Maintain, manage, and process client records and information in a confidential manner that meets HIPAA and other applicable confidentiality regulations. 
  • Adhere to EPU standards, policies, and procedures.
  • Providing translation of communication both verbal and written in Spanish as needed.
  • Standard operating hours of the EPU facility are Monday through Friday, 8:00 am – 5:00 pm. Position may require variation of hours depending upon department and position needs.
  • Perform other duties as assigned by the Director and/or Chief Executive Officer.

Requirements

Knowledge, Skill and Experience:

Education: Must have a High School diploma or equivalent. College course work preferred but not required. Bachelor’s degree encouraged.

Skills: Must have strong communication skills, both written and verbal; able to make good, logical independent decisions within guidelines, accept responsibility; be detailed oriented, dependable, and reliable, committed, highly motivated, and adaptable; have self-confidence, strong time management skills and a professional code of ethics and professional demeanor; and take pride in producing a high-quality product.

Experience:

  • Experience working as part of multidisciplinary team that assesses and links children with special needs and their families and referring them to community service providers.
  • Bilingual skills, Spanish required.
  • Proficient handling client records and information confidentially and in accordance with HIPAA and other applicable regulations.
  • Basic knowledge related to special education, educational strategies, assessment, conflict resolution and other processes related to accessing educational services.
  • Must be able to work with a diverse population and be a team player.
  • Skilled in obtaining confidential and sensitive information from parents in a compassionate manner.
  • Experience working as part of a team in implementing new and existing systems and quality assurance practices.
  • Must have experience working with Microsoft Office programs such as Word, Excel and Outlook.
  • Accurate data entry at the rate of 45 WPM or 11250 KPH.
  • Knowledgeable of community resources available to families/young children.
  • Hold a valid California Driver's license and good driving record with no serious violations for the previous two-year period and be able to provide proof of current automobile liability insurance.

Physical Demands:

Typical Working Condition: Position works from the EPU facility. The position requires the ability to sit and work on a computer for extensive periods of time. Position requires fingering (fine dexterity), repetitive hand and wrist motions.

Equipment Used: Computers, printers, telephones, and fax machines. May require operation of a vehicle for local travel.

Essential Physical Tasks: Must be able to sit for long periods of time, working with a keyboard, computer screen.
