We are looking for a mission driven Patient Care Coordinator to join our team! The Patient Care Coordinator (PCCO) contributes to the organization's overall goal to develop and implement a centralized (and shared) pharmacy that serves OneWorld Community Health Centers and the Network Clinic. The PCCO is responsible for patients' enrollment and re-enrollment in pharmacy assistance programs. Through computer-generated programs, they enter data, monitor the medication process, and monitor the patients participating in the assistance programs. They also serve as a liaison between the patients, their families, and the pharmacy staff. They provide interpretation and translation services as needed. The PCCO provides educational information to patients regarding medications, consults, and follow-ups all while respecting patients' rights and maintaining confidentiality. The PCCO will also assist the MTM (medication therapy management) pharmacy and participate in the specialty pharmacy programs team. They also provide administrative support within the pharmacy as needed.

This is a full-time position located on our main campus (30th & L). The schedule will be 40 hours per week; generally, Monday-Friday, 8:30am to 5:30pm.

QUALIFICATIONS:

- High School diploma or equivalent required.

- Current licensure as a Pharmacy Technician in the state of Nebraska is required.

- Must obtain National Pharmacy Technician Certification in the time allocated by Nebraska Pharmacy Laws.

- Strong computer skills in Microsoft Office products, particularly Excel, are required.

- Proficiency in both English and Spanish is required.

- Strong communication skills (oral and written) along with strong organizational skills are required.

- Demonstrated ability to think critically and be resourceful when troubleshooting.

- Demonstrated math skills including addition, subtraction, multiplication, and division along with analytical problem-solving experience.

- Demonstrated working knowledge of medication classes, types, and utilization.

- Demonstrated understanding and awareness of assistance programs, community resources, and the needs of the low-income and medically underserved population.

- Demonstrated ability to work with diverse groups in a coordinating role.
