Big Bend Hospice is hiring an experienced Advanced Practice Registered Nurse (APRN) to support our Transitions Supportive Care team.
We are looking for an APRN who can deliver quality patient-centered palliative care within the patient's home.
Travel Requirement: You will use your vehicle (mileage reimbursement applies) and travel to patients who reside in the 8-county service area.
This is a rewarding position where you will manage our patients' serious illnesses, discuss goals of care, talk about advanced directives, be an advocate and counselor for the patients and caregivers and so much more!

Main duties and responsibilities:

Patient Care
Provides medical care, palliates, and manages serious illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
Completes history and physical examinations by evaluating the patient's medical condition and health history, orders and interprets results from laboratory and diagnostic tests or procedures, diagnoses health conditions, and documents the findings in the patient's medical record.
Manages health conditions and serious illnesses by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals, and coordinating palliative/supportive care services.
Reviews clinical information, and coordinates and oversees patient care in consultation with the palliative/supportive interdisciplinary team (IDT) and other healthcare practitioners.
Ensures that patients receive appropriate measures to control symptoms through collaboration with interdisciplinary team (IDT) members.
Reviews and develops protocols for treatment; proposes options for interventions based on the literature with the collaborating physician.
Consults with the collaborating physician when the patient's plan of care is outside standardized practice and protocols.
Provides and maintains a safe environment for the patient/family.
Serves as an additional provider resource for Big Bend Hospice, completing face-to-face visits, addressing orders for medications and treatments and other hospice care-related needs, in line with APRN duties and responsibilities.
Participates in on-call rotation.
Communication
Communicates and collaborates with the interdisciplinary team (IDT) to create, review, revise, and document the patient's plan of care.
Attends and participates in palliative/supportive interdisciplinary team (IDT) meetings.
Completes and submits accurate, timely, and relevant clinical notes, physical exam notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record.
Consults with the patient's primary care physician, and other healthcare practitioners regarding the patient's eligibility for palliative/supportive care, ongoing care needs and medical management related to the patient's serious/terminal condition.
Communicates with other community health practitioners to coordinate the plan of care.
Educates and counsels patients, families, and/or caregivers re: preventative care, medical problems, psychological issues, symptom management, advanced care planning, and spiritual issues in conjunction with the interdisciplinary team (IDT) to meet the total needs of patients/families.
Assists the patient and family/caregiver and other team members in providing continuity of care.
Collaborates with the family/caregiver and the interdisciplinary team (IDT) to identify the goals of care and meet the care needs of the patient and family/caregiver.
Serves as a nursing resource for consultation and educations to members of the interdisciplinary team and other healthcare practitioners in the community. *
Establishes, builds, and nurtures relationships with staff and community to facilitate program growth.
Other duties as requested or assigned.
