Title: Registered Physical Therapist
Reports to: Program Manager/Director
Position Summary:
The Physical Therapist will report directly to the Program Manager who is responsible for overseeing direct clinical skills, documentation skills, and compliance to all professional standards. A review of performance will be conducted on a yearly basis.
Core Responsibilities:
The RPT is responsible for the evaluation and treatment of residents who demonstrate disorders of musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits.
Screening new residents within 24 hours of admission
Screening of long term residents upon receipt of a significant change form
Completing all documentation, including but not limited to: initial evaluation forms, weekly progress notes, monthly recertification forms, discharge summary, all other forms as designated by the facility and manager
Attending all required facility meetings and in-services
Coordinating with the program manager to develop programs
Assisting residents achieve their highest feasible level of functioning
Assisting the facility in meeting the standards necessary to obtain state, federal and other appropriate certification
Employ and follow all facility infection control policies and procedures including OBRA Resident Rights guidelines
Responsible for scheduling and completion of in-services for facility and staff education
Presenting yourself in a professional manner at all times including professional conduct and communication and appropriate dress while in the facility
Other duties as assigned by the PT Manager/Director and facility
Minimum Qualifications:
Bachelor’s of Science in Physical Therapy
