RN Outpatient Care Coordinator (Part-Time)
General Summary:
This Pilot project will help CHCWM better define the role of a care coordinator in the setting of subspecialties. Our goal is to better delineate care coordinators that partner closely with physician subspecialty teams, vs the role and responsibilities of helping manager the high risk patients undergoing therapy. In the era of value based care the need to expand the care coordinator role is inevitable and therefore needs better delineation of duties.
This role like all are care coordinator roles is an RN member of the multispecialty services team. Responsible for following patients throughout their trajectory of care. Ensures there is coordination of care within our organization as well as with outside healthcare providers.
Job Requirements:
Education: Licensed RN in the State of Michigan. Successful completion of competency based orientation program and chemotherapy certificate. ONS Chemo & Biotherapy Provider Card and CPR certification or acquisition within 90 days of employment.
Experience: Minimum of 3 years of clinical experience in oncology preferred.
Other: Demonstrates an ability to work independently in a self-directed environment. Very good time management and organizational skills. Effective written and verbal communication skills. Working knowledge of a variety of health care equipment. Ability to make quality, independent decisions. Ability to instruct and direct the work of others. 80% of time spend standing and/or walking. Ability to lift up to a 20-40 pound weight load. Very frequent lifting and bending, pushing and/or pulling loads. Able to work effectively and efficiently under tight deadlines, high volumes and multiple interruptions.
Preferred Knowledge:
BSN, OCN, 3 years of clinical experience in oncology
Essential Duties & Responsibilities:
  • Primary function is to support assigned provider.
  • Follows essential duties of care coordinators and partners with them in timely starting of treatments and to ensure appropriate follow ups are completed.
  • Sits with provider during clinic time and ensures timely submitting of orders and needed follow up
  • Checks on orders for timely completion such as molecular testing or referrals to other providers for additional needs or second opinions
  • Communicates labs/radiology results with provider and helps manage future treatment plan based off results
  • Reviews tentative new patient appointments and evaluates the urgency based off algorithm to guide new patient referral specialists. Uses provider input when necessary
  • Ensures the experience of patients is seamless despite the complexities of cancer care.
  • Evaluates the education and supportive needs of patients and families throughout their oncologic care. Partners with healthcare providers to make appropriate referrals.
  • Provides documentation consistent with documentation guidelines. Initiates and maintains communication with the patient and other healthcare team members to ensure the provision of care, continuity of care and coordination of services.
  • Maintains knowledge of current trends and developments in oncology by reading, attending conferences, seminars, etc.
Function as a team member by expanding his/her responsibilities as directed by management to facilitate optimal workflow within the cancer center.
Communicates and interacts with peers and other departments in a manner that is viewed as having a positive effect on patient services and CHCWM staff morale. Interacts effectively with patients/family members and demonstrates sensitivity in regards to cultural diversity, age specific needs and confidentiality.
This employee has access to medical records daily. The parts of the medical record s/he needs to references to perform his/her job are as follows: Electronic Medical Record (EMR).

Very fast-paced and ever changing healthcare environment. Demanding regarding deadlines and time frames. Very detailed and challenging work. Constant demand for updating knowledge. Works closely with physicians and ancillary personnel in caring for and treating oncology/hematology patients. Exposure to a variety of attitudes, personalities, and health related reactions of patients, families and visitors. Possible exposure to a variety of contagious diseases and/or hazardous materials.
