CFC Service Coordinator
Program: Child and Family Connections, Early Intervention.
Location(s): Arlington Heights, working with families in the area. Able to work from home a couple days a week.
Hours: Full time, Monday - Friday, 40 hrs / week.
Pay: $41,800 - $45,800 / year depending on education and experience, plus full -time benefits package.
Clearbrook is a non-profit, social services agency that specializes in providing innovative opportunities and services to individuals with intellectual and developmental disabilities.
The Service Coordinator assists and enables a child who is eligible for Early Intervention services and the child's family to receive rights, procedural safeguards, and services that are authorized under the State of Illinois' Early Intervention Program.
The responsibilities of a Service Coordinator include, but are not limited to:
Mandatory contact (by telephone, mail or in person) with the enrolled child/family at least one time per month
Coordinating the implementation of the service plan
Coordinating the completion of initial and annual evaluations/assessments
Facilitating and participating in the development, review and evaluation of the Individualized Family Service Plan (IFSP). This includes IFSP updates, six (6) month reviews and the annual evaluations of the IFSP
Mandatory distribution of the entire initial and annual IFSP, including evaluations/assessments, to all ongoing EI Providers before the start of the services.
Assisting families in identifying credentialed/enrolled/available EI Providers
Coordinating and monitoring (through the mandatory monthly contacts) the delivery of services identified in the child’s IFSP
Informing families of their rights and the availability of advocacy services
Facilitating the development and implementation of a transition plan to preschool or other services, if appropriate
Maintenance of the child’s comprehensive permanent record at the Child and Family Connections office.
Minimum Education / Experience Requirements:
Bachelor's degree or higher in human services, behavioral science, social science, or health related field.
Experience working with children (birth - age 3) with developmental delays.
Case management experience is preferred.
