Summary:

The Youth and Young Adult Case Manager is responsible for providing case management and supportive services that help Youth and Young Adults residing in Youth and Young Adult designated housing at HopePHL. The case manager will utilize a strengths-based approach to engage families in the development of a service plan tailored to meet their unique needs and will provide weekly coaching/counseling to facilitate goal attainment.

Essential Duties and Responsibilities:

· Provide case management services to youth and young adults residing in temporary, transitional housing and supportive housing with HopePHL

· Assist with the intake and orientation process of their new residence

· Assist residents in completing PA COMPASS, CHIP, SSI and/or other applications necessary to secure temporary cash/food assistance and health insurance for all family members. Ensure renewal applications are completed in a timely way

· Generate thorough written assessments for each individual which includes information gathered from HopePHL intake process, interviews with family members, and all documents received from other service providers.

· Engage family members, including fathers, in a problem-identification, planning and decision-making process which results in the development of a comprehensive family service plan.

· Collaborate with residents to identify and secure the resources needed to maintain housing and economic stability; ensure timely completion of renewal applications to maintain cash, food, utility, rental assistance, and health insurance for all family members.

· Using the comprehensive assessment and family service plan as the guide, case managers will meet regularly with families to link them to appropriate resources and service providers within the community and monitor utilization of services.

· Case managers will also monitor the quality of services families are receiving, offer solutions for eliminating service delivery obstacles, and make recommendations for additional linkages with other service providers as needed.

· Provide crisis prevention and intervention services as needed to preserve stable housing.

· Conduct home visits, assist with monthly financial planning, and coordinate as needed with property management.

· Organize workshops based on the needs of residents to increase social and resource networks.

· Refer residents to behavioral health services for screening and services as needed.

· Collaborate and communicate with HopePHL team members, and other service providers as needed to enhance service delivery; ensure continuity/consistency of care; and facilitate goal attainment.

· Document all case activity and family information in a timely manner in the internal and external database systems.

· Maintain audit ready files and conduct quarterly peer reviews to ensure compliance with program.

· Attend bi-weekly supervision on time and prepared with updated and accurate case files.

· Adhere to agency attendance policy and attend all required meetings.

· Share weekly schedule/calendar with Supervisor and communicate in a timely manner regarding any changes.

· Any concerns regarding the safety of clients must follow agency and program protocols.

· Complete and participate in professional development on an ongoing basis.

· Adhere to agency and professional code of ethics.

Required Education and Experience:
Bachelor’s degree in social work or a related human service field. Prior experience working with families.

Work Environment:

This position is onsite with some flexible schedule options. Occasionally travel to the offices of HopePHL and off-site locations will be required. The incumbent in this position will routinely use computers, photocopiers, filing cabinets and other standard office equipment. The position also requires you assist with large gift in-kind drives, and occasional event set up and breakdown.

Physical Demands:

While performing the duties of this job, the employee is frequently required to sit, talk and hear. The employee is occasionally required to walk, use hands and fingers to feel, handle, or operate objects or controls; and reach with hands and arms. The employee must occasionally lift and/or move up to 30 pounds.
