The STRONG Child & Adolescent Day Treatment program is a service of Christian Family Solutions Counseling Care & Services (CFS). We are recruiting a Discharge Planner and Community Resource Liaison at The STRONG Milwaukee Center, which is a National Health Service Corps (NHSC)-approved site. STRONG stands for Successfully Treating and Reaching Our Next Generation. The STRONG program recognizes the unique value of each child and that each child deserves a chance at a successful future.
This is a split role that will serve clients in our program with discharge planning and other case management functions, as well as build and maintain close contacts with outside organizations to develop outreach opportunities. Those who have a great desire for serving clients in various case management functions and a proven track record of developing beneficial community relationships are encouraged to apply.
This position is approximately 32 hours per week, Monday – Thursday between the hours of 7:30am-4:00pm, and is eligible for our comprehensive, full benefits package. For the right candidate, we may be willing to negotiate the number of hours per week and the responsibilities of this position, to meet our needs and those of the applicant.
What you will be doing:
You will be responsible for the case management of clients admitted and discharged from our program. Specifically, you’ll be working in the areas of attendance, discharge planning, arranging transportation, and admissions as needed. A heavy focus will be ensuring the quality continuum of care. This position will require various office support functions - answering phone calls, maintaining regular attendance spreadsheets, while facilitating effective case management. You will also be responsible for building case studies after successful treatment, and helping communicate STRONG’s positive treatment outcomes and impact with key stakeholders in the community. This includes identifying new opportunities for referral-generating relationships, developing new ideas for services, and promoting visits to STRONG, facilitating tours, meetings, written and audio/visual communication.
