Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals in need, helping them navigate through various challenges and access necessary resources. Your role will involve assessing client needs, developing care plans, and coordinating services to ensure the best possible outcomes.
Responsibilities:
- Conduct comprehensive assessments of clients' needs, including physical, emotional, and social factors
- Develop individualized care plans in collaboration with clients, focusing on their goals and strengths
- Provide counseling and support to clients, utilizing techniques such as motivational interviewing and addiction counseling
- Assist clients in accessing appropriate community resources, such as housing assistance (Section 8), behavioral health services, and workforce development programs
- Monitor client progress and make necessary adjustments to care plans as needed
- Collaborate with other professionals, including social workers, healthcare providers, and legal professionals, to coordinate services for clients
- Advocate for clients' rights and ensure they receive the support they need
- Maintain accurate and up-to-date documentation of client interactions and progress
Skills:
- Bachelor's or Master's degree in Social Work or related field
- Strong knowledge of social work principles and practices
- Experience in behavioral health or crisis management preferred
- Familiarity with Section 8 housing assistance programs is a plus
- Excellent communication skills, both verbal and written
- Ability to build rapport with diverse populations and establish trusting relationships
- Strong organizational skills and attention to detail
- Proficient in using computer systems for documentation purposes
