A DAY IN THE LIFE OF A CASE MANAGER
As a Case Manager, you provide direct assistance and case management to survivors of domestic violence. You work directly with survivors to determine areas of need, set goals, and develop a service plan. Additionally, you perform lethality assessments and crisis counseling and facilitate various support and life skills groups. You provide advocacy and general case management assistance related to legal, financial, medical, mental health, educational, employment, housing, and other social services for survivors and their children.
You facilitate linkages with and make referrals to community service providers to accomplish service plan goals. In order to assess the progress toward completing the service plan goals and identify areas requiring further assistance, you regularly conduct follow-up meetings with survivors. You are conscientious about keeping up-to-date and accurate documentation from which you compile statistical data in order to maintain compliance with established funding sources. Your job is highly collaborative, as you work closely with the case management team, community services, various advocates, and IFP attorneys to ensure effective coordination of services. QUALIFICATIONS FOR A CASE MANAGER
Are you organized and able to effectively prioritize multiple tasks? Do you have excellent communication skills, both written and verbal? Are you patient and calm but quick to respond? Do you have good judgment? Are you conscientious about following policies and procedures and discreet with confidential information? Can you show empathy and compassion while maintaining professional boundaries? If so, you might just be perfect for this Outreach Case Manager position!
