We are seeking a compassionate and dedicated Homeless Outreach Team Member to join our organization's efforts in providing support and resources to individuals experiencing homelessness. As a vital member of our outreach team, you will work directly with individuals living on the streets or in temporary shelters, helping them access essential services, navigate resources, and find pathways to stable housing. This position requires strong interpersonal skills, a non-judgmental attitude, and a genuine commitment to making a positive impact in the lives of the homeless population.

Responsibilities:

  • Build trusting relationships with individuals, actively listening to their stories, concerns, and needs.
  • Conduct needs assessments and collaborate with clients to develop individualized plans to address their immediate and long-term needs.

Resource Navigation:

  • Provide accurate and up-to-dateinformation about available community resources, such as emergency shelters, meal programs, healthcare services, substance abuse treatment, mental health support, and job training programs.
  • Assist individuals in accessing and navigating these resources, including making referrals and accompanying clients to appointments when necessary.
  • Help clients complete necessary paperwork and applications for benefits, housing, and other support services.

Case Management:

  • Maintain detailed and confidential case records, documenting client interactions, services provided, and progress made.
  • Conduct regular follow-up visits and check-ins to assess client needs, monitor progress, and provide ongoing support.
  • Collaborate with internal team members and external service providers to coordinate comprehensive care and ensure continuity of services.

Crisis Intervention and Support:

  • Respond to crises and emergencies as they arise, including situations involving mental health crises, substance abuse, violence, or medical emergencies.
  • Liaise with appropriate emergency services, such as police, paramedics, or social workers, when necessary, to ensure the safety and well-being of clients.
  • Offer emotional support, advocacy, and crisis de-escalation techniques to individuals in distress.

Community Engagement:

  • Foster positive relationships and collaborate with local community organizations, businesses, and government agencies to advocate for the needs of individuals experiencing homelessness.
  • Participate in community outreach events, workshops, and public education initiatives to raise awareness and reduce stigmatization surrounding homelessness.
  • Provide training and education to community members on how to best support individuals experiencing homelessness.

Qualifications:

  • Bachelor's degree in social work, psychology, human services, or a related field (or equivalent experience).
  • Previous experience working directly with individuals experiencing homelessness, substance abuse, mental health challenges, or other vulnerable populations.
  • Strong interpersonal and communication skills, with the ability to build rapport and establish trust with diverse individuals.
  • Knowledge of local resources and social service agencies available to individuals experiencing homelessness.
  • Ability to work effectively in a fast-paced, unpredictable environment with a high degree of flexibility and adaptability.
  • Understanding of trauma-informed care principles and a non-judgmental approach to serving marginalized populations.
  • Empathy, compassion, and a commitment to advocating for social justice and equity.
  • Valid driver's license and reliable transportation to travel to various outreach locations.
