Dept of State Health Services is Hiring a CSHCN Program Coordinator Near Austin, TX

Performs advanced consultative services and technical work related to the planning, development and implementation of systems of care for children and youth with special health care needs (CSHCN) and their families. Works to coordinate, develop, and carry out the Title V Block Grant State Action Plan for CSHCN and performs associated planning and collaborative activities with other agency programs, organizations, and groups that address issues pertaining to CSHCN. Serves as program liaison to CSHCN Systems Development Group community-based contractors. Performs quality assurance monitoring; documents findings and prepares evaluation reports. Researches and prepares evaluation and development of grant opportunities, contracts, fiscal impact reports, and other reports and studies. Works under limited supervision, with considerable latitude for initiative and independent judgment.
Essential Job Functions:
Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
(40%) Provides consultation and technical assistance to plan, implement, and monitor statewide children and youth with special health care needs activities. Develops and carries out objectives, strategies, and evidence-based or informed measures; monitor staff and contractor activities related to assigned initiatives. Oversees the planning, implementation, and evaluation of activities related to assigned initiatives, including development of program evaluation, administrative rules, policies, and procedures; collects, analyzes, reports, and presents data. Develops and reports on activities in support of the Title V performance measures, health status indicators, and outcome measures.
(25%) Leads the generation of ideas and design methods for program quality improvement initiatives. Identifies needed areas of change and makes recommendations/leads implementation of techniques to evaluate and improve program activities, including contractor activities. Assesses and addresses ongoing educational needs of staff relative to children with special health care needs. Collects, organizes, analyses, and prepares material in response to requests for information and reports.
(15%) Establishes effective partnerships and collaborative workgroups with other state agencies, disability service/advocacy organizations, consumer and provider groups, funding entities, and internal partners at DSHS to further the awareness of the needs and issues of children, youth, and adults with special health care needs and their families and collaboratively plan activities to address Title V CSHCN Performance Measures. Represents the CSHCN Systems Development Group, Maternal and Child Health Section, and Department of State Health Services at meetings, hearings, conferences, and seminars or on boards, panels, and committees. Promotes use of evidence-based practices. Seeks opportunities to integrate services for establishing medical homes, health care transition, and community inclusion for CSHCN and their families.
(15%) Serves as program liaison to CSHCN Systems Development Group community-based contractors. Collaborates with program staff in the application review and/or contract renewal process. Collaborates in the development of evaluation tools; reviews and evaluates service delivery methods, outputs, and activities; identifies gaps in resources; and makes recommendations for improvement and funding based on review criteria. Performs quality assurance monitoring; documents findings and prepares evaluation reports. Researches and prepares evaluation and development of grant opportunities, contracts, fiscal impact reports, and other reports and studies.
(5%) Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
Knowledge Skills Abilities:
Knowledge of issues and public health principles and practices pertaining to children with special health care needs and adults with disabilities and their families.
Knowledge of principles of monitoring and evaluation; benchmarking; development of evaluation tools.
Knowledge of local, state, and federal laws and regulations and current trends relevant to children with special health care needs and adults with disabilities and their families.
Knowledge of program development, planning, and implementation.
Knowledge of state and federal programs, contract/performance monitoring, and grant monitoring, evaluation, and budgeting processes.
Knowledge of and experience with coordination of agency/ program services and interagency planning for integration of service systems to improve quality of health care.
Skill in using computer software applications (Microsoft Word, Excel, and PowerPoint) for word processing, reports, and presentations of narrative and financial information and analysis.
Skill in communicating information clearly, concisely, and effectively verbally and in writing.
Skills in project development, planning, funding, management, and evaluation (including grant-writing).
Skill in interpreting data.
Ability to function as a member of a team.
Ability to work collaboratively and productively with people from various professional, socio-economic, and cultural backgrounds.
Ability to oversee program activities and to establish goals and objectives.
Ability to prioritize, schedule, and monitor completion of complex assignments.

Initial Selection Criteria:
Graduation from an accredited four-year college or university in a relevant field. Master’s degree preferred.
Experience working in public health, maternal and child health, and/or health care policy development.
Experience developing, writing, and presenting reports and communications.
Experience overseeing the coordination of projects.
Experience in data analysis and evaluation preferred.
