General Description:

We are seeking a talented and personable nurse to fill the Clinical Assessment & Eligibility Nurse role (CAE) to join our nursing department. The primary responsibility of this role is to conduct personal care assessments. The candidate will be responsible for evaluating individuals already in a nursing home. This position requires a detail-oriented and compassionate approach, focusing on upholding the highest standards of care and adherence to state guidelines.

Requirements:
  • Conduct thorough and compassionate assessment of the Older adult's physical, emotional, and social needs.
  • Evaluate consumers' eligibility for various programs based on established criteria.
  • Coordinate with healthcare providers and community resources to ensure seamless service delivery.
  • Maintain accurate and confidential assessments, care plans, and consumer interaction records.
  • Conducts weekly SNF CAE screenings as required.
  • Provides nursing consultation for the Community Transition Liaison Program and participates in discharge planning meetings as appropriate.
  • Serves as the primary contact for the Department of Development Service (DDS) and Department of Mental Health (DMH) regarding CAE screenings and nursing facility residents
  • Builds and maintains collegial environment that supports positive group processes.

Qualifications and Education Requirements

  • Graduate of an accredited school of nursing, with current MA nursing license
  • Minimum three (3) years of nursing experience required; community experience preferred.
  • Excellent computer, organizational, interpersonal, written, and verbal communication skills are essential
  • Experience and interest in the elderly population and/or community health.
  • Candidate possesses good communication, organization, and time management skills
  • Knowledge of basic medical, mental health, and social service delivery systems and acceptable record-keeping practices is essential.

Essential Functions:

  • Sufficient clarity of speech and hearing which permits communication with supervisor, co-workers, consumers, community professionals, and general public.
  • Ability to talk on a phone, stand, walk, sit, and travel in the community.
  • Ability to travel throughout the state of Massachusetts in a private vehicle under all weather conditions including snow.
  • Ability to express ideas concisely and clearly, orally and in writing.
  • Sufficient vision which permits production and review of a wide variety of materials, written correspondence, reports and related electronic and paper communication.
  • Ability to lift and carry objects weighing 20 pounds.
  • Ability to read and analyze data.
  • Ability to exercise sound judgment in making decisions.
