This position is contingent and paid on a per visit basis; orientation and additional training will be paid at a separate hourly rate. We offer competitive wages, bi-weekly pay, and direct deposit.

Responsibilities:

• Provide initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments.
• Perform assessments within 24-48 hours of receiving the initial referral.
• Establish a treatment plan in consultation with the physician and revise it with the approval of the physician as necessary.
• Perform treatments as outlined on the plan of care and as directed by the physician.
• Educate patients and caregivers in a way that they understand.
• Record on a timely basis all evaluation data, treatments, and patient's response to therapy intervention.
• Participate with staff, patients, and physicians in discharge planning activities.
• Participate in case conferences and other meetings as necessary.
• Supervise LPNs as per agency policy.
• Adhere to Federal, State, and Accreditation requirements including Medicare and Medicaid regulations.

Minimum Qualifications:

• Registered Nurse License in good standing within the State of Michigan
• Must have a minimum of two (2) years of nursing experience, preferred home care.
• Must have knowledge of Medicare/CMS guidelines.
• Must have experience with Outcome and Assessment Information Set (OASIS) assessments.
• Experience using an EMR (Kinnser preferred)
• Must have your own reliable transportation, valid MI driver’s license, with proof of auto insurance.
• COVID-19 Vaccination Required
• BLS through AHA or Red Cross

Job Types: Part-time, PRN

Pay: $60.00 - $110.00 per hour
