As an Occupational Therapist, you will use your knowledge to create individualized care plans to improve patient outcomes and independence. No Place Like Home Health is seeking Occupational Therapists who are dedicated to creating a professional, diversified work environment, while building relationships with our amazing clientele. This position is contingent and paid on a per visit basis; orientation and additional training will be paid at a separate hourly rate. We offer competitive wages, bi-weekly pay, and direct deposit.

Responsibilities:

  • Provide initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments if required.
  • Perform assessments and visits within the required time frame.
  • Establish a treatment plan in consultation with the physician and revise it with the approval of the physician as necessary.
  • Perform treatments as outlined on the plan of care and as directed by the physician.
  • Educate patients and caregivers in a way that they understand.
  • Record on a timely basis all evaluation data, treatments, and patient's response to therapy intervention.
  • Supervise Occupational Therapist Assistants as per agency policy.
  • Participate with staff, patients, and physicians in discharge planning activities.
  • Adhere to Federal, State and Accreditation requirements including Medicare and Medicaid regulations.

Minimum Qualifications:

  • Occupational Therapist License in good standing within the State of Michigan
  • Preferred one (1) or more years of OT experience, specifically in a community/home health care setting.
  • Must have knowledge of Medicare/CMS guidelines.
  • Experience using an EMR (Kinnser preferred)
  • Must have your own reliable transportation, valid MI driver’s license, with proof of auto insurance.
  • COVID-19 Vaccination Required
  • BLS through AHA or Red Cross

Job Types: Part-time, PRN

Pay: $30.00 - $40.00 per hour
