We are seeking a highly motivated MDS Coordinator to join our team at Seneca Senior Living! The MDS Coordinator will be responsible for coordinating and overseeing the completion of assessments for all residents in accordance with state and federal regulations.
The successful candidate will also manage the care planning process and ensure that all documentation is accurate and up-to-date.
Responsibilities :
- Coordinate and complete assessments for all residents in accordance with state and federal regulations.
- Ensure that all assessments and documentation are completed in a timely and accurate manner.
- Communicate with physicians, therapists, and other healthcare providers to ensure that the care plan is being followed.
- Maintain a working knowledge of state and federal regulations related to long-term care. Requirements :
- RN required
- RN required
- Minimum of 2 years of experience in long-term care and MDS coordination.
- Strong communication and interpersonal skills.
- Excellent organizational and time-management skills.
- Ability to work independently and as part of a team.
- Knowledge of state and federal regulations related to long-term care.
