Bert Nash Community Mental Health Center is Hiring an ACT Co-Occurring Disorder Specialist, Case Manager Near Lawrence, KS

Salary is commensurate with experience within a range of $20.00-$27.68 This position is full-time, non-exempt and eligible for our benefits. A.
Responsibilities and Duties: The primary responsibility of the Disorder Specialist Case Manager is to coordinate services for clients who meet the criteria for the targeted population. Specific duties include:

1. Help clients/and families to identify, assess, and understand the symptoms of their mental illness and to minimize the effect of those symptoms in all the significant areas of their lives.

2. Assist clients in learning, practicing, and implementing new skills that contribute to their recovery as specified in the treatment plan.

3. Provide supportive counseling, solutions focused interventions; behavioral analysis to increase clients ability to function in all life domains.

4. Provide outreach and crisis intervention services to clients and prospective clients to enable timely access to those services appropriate and necessary for crisis stabilization with the purpose of diverting the client from hospitalization with the goal of allowing the client to maintain independent living;

5. Work with identified team member, including the client and/or family, to develop an person centered plan of care, that is individualized to meet the identified needs and goals of the client

6. Complete strengths or other assessments, as appropriate, with client and identify strategies or treatment options associated with the consumer's mental illness with the goal of minimizing the negative effects of mental illness symptoms or associated environmental stressors.

7. Develop individualized crisis plan with client and review during treatment plan review time.

8. Coordinate clients' care services with other providers, family, and community collaterals primarily with the client consulting in the process.

9. Provide services hours based on the level of care identified compliant with established BNC standards of service delivery (1125 hours per year)

10. Deliver services based on the appropriate intervention, ie. CPST vs PRI vs TCM

11. Complete all paperwork associated with the financial, clinical, legal or regulatory management of the case accurately and in a timely manner

12. Complete clinical notes within 24 hours

13. While operating in your role, represent Bert Nash by demonstrating respect, collaboration, cooperation with all community partners in public.

14. Participate in strategic planning, program development and performance improvement processes

15. Additional responsibilities that may be assigned as circumstances warrant specifically related to supportive housing facilities and clients; and

16. Regular and predictable attendance is an essential function of the job.

B. Accountability: The Case Manager will be directly accountable to the Program Manager or his/her designee. The Program Manager or his/her designee will, no less than annually, complete an evaluation in compliance with personnel policies.

C. Qualifications: The Case Manager position requires:

1. a BA/BS degree or four years of equivalent education and/or experience working in the human services field.

2. Reliable transportation; and a valid, unrestricted driver's license and proof of current car insurance

3. Prior computer experience and basic computer fluency.

4. Must be able to achieve within one's first 6 months of employment a working competency in the management and use of the Center's electronic medical record (EMR) software as demonstrated by timely and accurate documentation of the clinical record.

5. An ability to work independently and in cooperation with others.

6. Ability to exercise sound independent judgment

7. Strong organizational skills

8. Strong oral and written communication skills

9. Strong interpersonal skills in working with diverse publics

10. Flexibility in assigned working hours

11. A commitment to interdisciplinary and community mental health center approaches

12. An energetic and positive approach to the rapidly evolving changes and challenges of a complex work place.

Bert Nash Center requires proof of Covid-19 immunization prior to starting any position. Covid-19 vaccine series must be completed or in process to be cleared for new hire orientation.
