Description

Position Overview

Reporting to the VP of Home Care, the Director of Clinical Support is responsible for the day to day operational management of the paraprofessionals, clinical triage, telemonitoring staff and virtual visit nurse teams. Responsibilities include supervision and coordination of all aspects of Paraprofessional services. Provides leadership by developing and implementing strategies that will improve the delivery of services throughout the entire organization.

Essential Functions

  • The Director of Clinical Support provides clinical supervision oversight to all personnel in the Paraprofessional Program.
  • Works closely with the Paraprofessional Administrative Manager.
  • Serves as a member of the Agency's Management Team and assists with development, implementation, maintenance and revision of policies, objectives and goals.
  • Monitors the performance of paraprofessional staff, clinical triage, telemonitoring staff and virtual visit nurses identifying individual strength and weakness along with assisting staff in setting related performance and learning goals.
  • Supports staff development by working with the Education Team and participating in orientation and annual education programs, including validation of competency and skills testing requirements in accordance with Agency policy.
  • Develops standards and procedures for providing home health care and for evaluating service.
  • Actively participates in QAPI program and shares outcomes and correction plans with staff.
  • Participates in hiring process for paraprofessional staff, triage staff and virtual visit nurse staff, including interviews, recommendations for hire as well as employee retention, support and mentoring.
  • Assures compliance with all state and federal laws regarding licensure and certification of the agency. Maintains knowledge of the Medicare Conditions of Participation for Home Health and an understanding of Hospice Conditions of Participation.
  • Works to resolve agency communication issues that can impact patient care.
  • Attends scheduling meetings. reviews the agency's minimum paraprofessional staffing needs with the Director of Operations and Scheduling Manager when approving vacations and other time off requests if requested.
  • Assists staff in planning and managing work to ensure that time assigned meets the needs of the patient and is realistic.
  • Manages and supports phlebotomy staff to ensure accurate completion of lab slips, use of correct tubes for the draws as ordered and communication to the appropriate staff member. Ensures that the phlebotomy staff completes annual competencies. Establishes quarterly meetings with phlebotomy staff to review any changes related to the service and competencies with equipment.
  • Investigates and reviews patient care problems/complaints/incidents with staff to identify reasons for concerns. Provides staff with feedback on findings along with recommending improvement activities when findings are identified. Completes and investigates any occurrence reporting.
  • Acts as a Laboratory Director and provides clinical oversight.

Other Job Functions

Demonstrates flexibility in the role by accepting other duties as assigned for which qualified.

Requirements

  1. Graduate of an accredited school of Nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC)
  2. Bachelors in Nursing or Physical Therapy; Masters in health-related field preferred
  3. One to two years of clinical experience (community health or hospital practice)
  4. Three to five years of management experience
