SummaryOur most successful Patient Navigator candidates will be energetic, engaging and deeply compassionate leaders who inspire their team to follow their lead. They’ll be people who flow easily with what others call stress, with being on call at a moment’s notice to meet people during an important time in their lives.

They’ll be passionate about the impact their work makes, encouraging and inspiring their staff of Patient Engagement Specialists to always perform to their best capacity.
Responsibilities
- Assess patient out of facility biopsychosocial stressors (i.e. housing, financial, legal, medical, employment, etc.) and developing a strategy to meet these needs during and after treatment episode
- Facilitate multiple care aspects (case coordination, information sharing, aftercare planning, court related issues)
- Maintain a good working relationship with support networks, government resources, community resources, employers, and referral sources
- Complete of all necessary documentation related to FMLA and Short-Term Disability
- Maintain patient records by reviewing and completing case notes
- Prepare patients’ discharge by reviewing and coordinating community services necessary to maintain sobriety outside of structured setting
- Participate in weekly multidisciplinary treatment team as part of ongoing assessment of patient needs and plan for discharge
- Be involved in daily census management including coordinating discharge schedule and communicating to treatment team and administrative team
- High School Diploma or equivalent
- 1-year prior experience working with Substance Abuse populations strongly preferred
- Previous Case Management experience strongly preferred
- Valid, unrestricted driver’s license
- Clean driving record for at least 3 years
- Must be committed to drug-free lifestyle and responsible alcohol use
- Personal passion or interest in substance abuse issues strongly preferred
