40 HPW 1ST PRIMARILY MON-FRIMUST BE FLEXIBLE DEPENDING ON INDIVIDUALS NEEDS
JOB DUTIES:
- Lead liaison for continuity of care for medically complex individuals in life sharing family home.
- Travel is required. Area would include Erie, Crawford, Venango counties.
- Take point on collaboration of individual’s support team.
- Manage treatment plans and provide medical training to family members.
